Holland Bloorview: Hospital to Home for Children with Medical Complexity

Please note: this activity is only open to students currently or who have recently completed a placement at Holland Bloorview Kids Rehabilitation Hospital.

Children and youth who have complex medical needs often transition between hospitals and then, following their hospital stay, they are discharged home. Children, youth, caregivers and family members benefit greatly from the support of an interprofessional team who work collaboratively with each other and with the children, youth, caregivers and family members to ensure a safe and successful transition home, maximize integration into the community and enhance their quality of life.

Learning Objectives
By the end of the session, learners will be able to:
1) Describe the lived experience of a client and family transitioning home;
2) Identify the strengths and challenges of transition to the community; and
3) Outline the value of having an interprofessional team support a client and family’s transition journey home.
